How It Works
Once your client has received their approval from My Aged Care to receive Restorative Care services, simply make a referral via our website or call us on 1300 851 639, and our team will:
- Conduct a thorough assessment of your clients strengths, challenges, and goals.
- Design a personalised care plan, focused on restoring function and supporting your clients independence.
- Coordinate and deliver services either in-person or via telehealth, from physiotherapy and nursing, to occupational therapy, nutrition support, and home modifications, we offer a truly multidisciplinary approach to maximise client outcomes.
- Regular progress updates and comprehensive discharge planning ensures that you as the provider and your client have a clear understanding of how to manage their health following the successful completion of their therapy.
