Backed by the latest research, our 12-week Healthy Joints Program aligns seamlessly with Restorative Care pathway principles and is designed to restore joint function, improve mobility and independence, and support participants to engage fully in daily life without excessive pain or stiffness. Led by expert physiotherapists, this evidence-based program delivers targeted, individualised exercise alongside essential education to help participants effectively manage osteoarthritis of the knee, hip, or spine, as well as other chronic pain conditions. Build strength, reduce discomfort, and regain confidence—empowering long-term activity and independent living.

Program Overview

A 12-week multidisciplinary program combining targeted exercise and education to improve function, reduce symptoms, and build independence for participants with heart or lung conditions, offered in group or one-to-one formats. Through tailored exercises and evidence-based education, your participant will learn to breathe easier, move better, and take control of your health. Achieve lasting results in a supportive, restorative environment focused on your goals and long-term wellbeing.

Ideal Program Candidate:

  • Osteoarthritis/ rheumatoid arthritis – hip, knee, shoulder, spine
  • Degeneration & other conditions of the spine
  • Chronic back pain

Program Goals & Outcomes:

  • develop and improve practical disease self-management strategies
  • education on topics such as medication, exercise, diet, weigh management to maximise function
  • improved mobility, social participation & engagement
  • educate on task modification, review for aid & equipment to maintain/ improve independence with personal and daily living tasks

Program Inclusions

 

Assessment:Comprehensive multidisciplinary individualised assessments with clearly defined SMART goals that align with the participant’s goal plan and desired functional outcomes (2hr assessment & report in person)
Program Discharge:Exit plan outlining goals achieved during the program, services received and multidisciplinary team recommendations for functional requirements post program (2hr reassessment & report in person)
Service Evidence:Record of attendance/ goal attainment & progress report
Program Delivery:All options run for a period of 12-week duration
Additional Inclusions:

Individualised home exercise program and self-management education

Tea & coffee provided post session (centre-based sessions only)

An ideal choice for individuals who find it challenging to exercise independently, want expert guidance and motivation, or are eager to maximise their results with professional support.
